Do you need to undergo a medical procedure? |

Step 1: Get to know us… Read the “Why Mentor Medic” and “About us” pages, and of course our FAQ.
Step 2: Complete the online "Details Request" Application Form, or contact a Mentor Medic representative.
Step 3: Provide us your medical information Your Mentor Medic representative will work with you and your physicians at home to relay medical data, including general medical history (2-3 pages), health record, test results, lab reports, MRI, x-rays, pathology reports from previous treatments, consultation records, immunization history and other documentation. In order to better facilitate this process, we will need to receive digital copies of these documents. If you do not have this information in digital format, you can use fax as well.
Step 4: Passport / Visa Acquisition If you do not already possess an updated Passport or Visa (if required), Mentor Medic representative will assist you with the acquisition of these documents, and locate the appropriate embassy for service. Please scan your (and your companion) Passports (and Visas), and email / fax to us.
Step 5: Receive a price quotation for the procedures and accommodations : During this process, you will receive a price quotation for the procedure(s) and accommodation. Also, you'll be provided with a host of information about the procedure(s), hospital, doctors, etc. You'll be able to review the price quotation and assess potential dates to travel. At this point, we will discuss the issue of your procedure(s) and accommodation, and of course talk about the dates you wish to plan your medical procedure(s). Once you have reached a decision and chosen the dates, we will check with our selected hospital affiliate to see if these dates are available. If the pricing is acceptable to you, we can then move on to the next step of executing your medical travel request.
Step 6: Mentor Medic will do the rest for you… Arrival in destination and greeted by your Program Manager (PM) or Designated Driver. If you've opted for a PM, he/she will be your personal assistant throughout your stay. He/she is fluent in your language and will be there for you every step of the way. This person will accompany you to all your doctor’s appointments and medical treatments. If requested, you will be given a cell phone upon your arrival so that you can contact your PM at any time. It is important for you to know that we have carefully selected Mentor Medic's PM’s. These PM’s are professionals with extensive experience in the medical tourism business, as we understand that our reputation is in their hands. |
